Confidentiality and Privacy Statement

To ensure the protection of your personal and health information, completion of a confidentiality release form is required. This form enables you to specify the type of information to be shared and the length of time for which consent is granted.

All information, documentation, and data submitted during the evaluation and consultation process will be handled in accordance with the Health Insurance Portability and Accountability Act (HIPAA) and the Family Educational Rights and Privacy Act (FERPA), as applicable within the public school system. This information will be used solely for the purposes of providing comprehensive evaluation services, ongoing communication, and follow-up consultations.

By submitting the required form(s), you acknowledge and consent to the collection, use, and analysis of your information for these specified purposes. All evaluations are conducted following the ethical standards and administration protocols established by test publishers and professional guidelines. Results will be reviewed, summarized, and included in the final evaluation report, as well as discussed during any follow-up consultations.
